Service Learning Permission Form
AMERICAN INTERNATIONAL SCHOOL/DHAKA
Student’s Service Learning Group:
Sites/places to be visited:
I give permission for my child to participate in AIS/D High School Service Learning on-site visits and other planned activities.
I understand that his/her participation is sanctioned by the school as a curricular related activity that requires my child to perform 10 hours of service to their Service Learning project. Most of the 10 hour requirement may require my child to visit the off campus site of their community service organization. I understand that my student may choose to do more than the required hours to further benefit the Service Learning group goals.
On-site visits and other planned activities performed outside of normal school hours may subject my student to the normal risks associated with transport in Dhaka and the surrounding areas. I understand that the school will provide reasonable supervision, or arrange with site personnel to do so, for these activities for my child while on Service Learning projects. When transported by school vehicles, all the normal school rules will apply.
In the event that the site visit is sanctioned by the school but made using a private vehicle, the school will bare no responsibility for my child’s safe conduct to and from the site. In such cases the private vehicle owner will be held accountable in case of accident or harmful event.
In signing this permission slip, I giving permission for my child to be transported via school vehicle to activities pertaining to his/her Service Learning group for the school year 2010-2011. I am also taking responsibility to ensure that my student is travelling safely should I agree for him or her to be transported via private vehicle to a project site. In this circumstance, I assume all liability and shall hold AISD harmless.
